Vehicle Revenue License
Revenue License for vehicles are issued between 9.00 a.m. to 3.00 p.m. only.
Documents to be furnished for obtaining a Revenue License for vehicles
01.Certificate of Vehicle Registration (Book) – The relevant vehicle should belong to the North Western Province.
- In case of Vehicles owned by Public Institutes, Finance Companies, and Banks a certified copy of the certificate of registration issued by the particular institute should be furnished. (Absolute ownership is required for certification and this is valid only for a period of 03 months from the date of issue.)
- In case the certificate of registration is not available an abstract certified by the Commissioner of Motor Traffic is required (MTA – 11) instead.
02. Valid Insurance Certificate at the effective date of license.
03. Revenue certificate obtained for the last time.
04.In case of Buses, Lories, Ambulance, Hearse, Three Wheelers, Vans and Lorry Trailers original copy of the certificate of eligibility obtained by an approved garage, cash receipt, or a photocopy of the cash receipt. (This is valid only for a period of 02 months from the date of issue)
05. In case of Omni buses the original copy and a photocopy of the valid passenger transport permit issued for the year concerned.
06. Certificate of emission of gases (in case repaired vehicles from the date of registration and in case new vehicles from the effective date of revenue license since the second year). This certificate will be valid for obtaining the revenue license only for a period of one year.
Obtaining a duplicate of the license :-
- A certified copy of the complaint lodged at the Police should be furnished together with the documents specified under No.01, and 02.

Timber cutting permits
Documents to be furnished :-
- Letter of request

- Deed of the land (certified by the Grama Niladhari)
-Folio – (Should have obtained within 03 months from the date of certification by G.N.)
- In case a joint land the letter of consent of the partners (should have certified by the G.N.)
- Life Interest or Deed of Gift – In case Deed of Gift consent of the grantor
- In case the owner was deceased documents to verify his/her death (Death/Marriages/Birth certificates)
-If the owner was deceased letters of consent of the spouse and children (should have certified by the Grama Niladhari)
-If trees are felled for a new construction the plan of the new construction
- In case electricity wires are obstructed letter of notification
- In case of felling risky trees letters of notification to the Pradeshiya Sabha/ Urban Council
- In case of a land involving court decisions orders issued by the Court
-In case a tree located at the boundary of a fence letter of no – objection from the immediate owner of the land certified by the G.N.
- In case of felling trees in a land exceeding 12.5 Acres in extent Environment Report is required.
Timber transportation licenses permits
Letters to be furnished:-

- Grama Niladhari Report Timber Logs, sawed timber or Furniture
- Permit issued for felling Jack/Rata Del/ Female Palmyra
-Register of blocking timber certified by the G.N.
- Recommendation of the Forest Department

- In case issuing permits for re transport of Timber/wooden furniture transported by the Train the original permit and the receipt issued by the Department of Railway.
Permits for animal transportation

- Certificate of animal health (should be obtained by the relevant veterinary office)
- Certificate of recommendation of vehicles
- Cattle Voucher
- Report of the Grama Niladhari where animals are living
- If animals are caries away from the Divisional Secretariat a letter of Grama Niladhari of the relevant division in the said Divisional Secretariat Division (to the effect that the adequate space is available for rearing animals and the person concerned is resided in the said Division) countersigned by the Divisional Secretary.
A fee of Rs. 50/- per each animal is levied for transport

Firearms permits
Documents to be furnished in respect of obtaining a license for possession of a gun in terms of Section 22 of Firearms Ordinance
(For a firearm kept in possession for “Agricultural Purpose”)

Documents to be furnished for transfer of firearms in terms of Section 15 of Fire Arm Ordinance
(For a firearm kept in possession in respect of "Security of property")
A fire arm is transferred only for a family member of the person who possesses a firearm. A Family member means the husband, wife, son/ daughter or married spouses of them.
1. Perfected application of transfer of firearms and a copy of National Identity Card.
2. Certified copy of National Identity Card of the Transferee.
3. Certified copies of relevant birth certificate/marriage certificate for confirmation of relationship.
4.Businesses (Business Registration certificates, Particulars of tax payments and Bank Statements) owned by the transferee of firearms).
5.If the relevant property has not been transferred to the transferor the affidavit to the effect that affirming the said property will be transferred to the transferee by the owner in future and certified photocopies of the particular business.
6.In case there are children other than the transferee the affidavit which conveys their consent in respect of transferring the business to the transferee.
7. In case there are village threats the police report in that regard.
8.In case the transferee is a member of the Three Forces or the Police a letter of concurrence in respect of transferring the firearm from the Head of the Department where the person is employed.
9.A Photocopy of annual license obtained for the last time in respect of the firearm expected to be transferred.
10.In case the owner of the firearm is dead a certified photocopy of his/her death certificate.
11. The affidavits should have been written correctly so as to prove the relevant facts as well as stamps to the prescribed value should be pasted and signed. Similarly, the frank of the Justice of Peace or Commissioner for Oaths who takes the oath should be clearly stamped and signed appropriately so as to recognize the person obviously.
Documents to be furnished for transfer of firearm in terms of Section 15 of Firearm Ordinance
(For a firearm which is kept in possession for "Agricultural Purpose")

2. A certified copy of the National Identity Card of the transferee.

4. A certified copy of deeds of agricultural lands transferred to the transferee of firearms.
5.In case the agricultural lands have not been transferred through deeds the affidavit to the effect that affirming the said lands will be transferred to the transferee by the owner in the future and the certified copies of deeds of lands. (the deed numbers and respective extents of land area (Acre, Rood, Perch) should be stated in the aforesaid affidavit).
6.In case there are children living other than the transferee the affidavit of them stating their consent in respect of transferring the agricultural lands to the transferee.
7. What are the crops cultivated at the lands depicted in the deeds according to the above Para No. 3 and 4? What is the extent of those lands? A certificate describing the damages caused to those crops issued by a divisional level officer in the relevant field.
8. In case the transferee is a member of the Three Forces or the Police a letter of concurrence in respect of transferring the firearm from the Head of the Department where the person is employed.
9. A Photocopy of annual license obtained for the last time in respect of the firearm expected to be transferred.
10. If the owner of the fire arm is dead a certified photocopy of his/her Death Certificate.
